1. Agreement and acceptance
This Affiliate Agreement (the "Agreement") is entered into between Connect Med ("Connect Med", "we", "us") and the individual or entity that enrolls in the Connect Med Affiliate Program ("Affiliate", "you"). By completing enrollment, accepting the enrollment checkboxes, or using an assigned Affiliate ID or referral link, you accept this Agreement and all policies referenced in it.
Connect Med may update this Agreement. Material changes are communicated through the partner dashboard or by email. Continued participation after an update constitutes acceptance.
2. Program participation and cost
- There is no join fee and no monthly fee to participate as an Affiliate.
- Enrollment is subject to approval. Connect Med may decline, suspend or terminate any enrollment at its discretion.
- Each Affiliate receives a unique Affiliate ID and referral link used for attribution. You may not share, resell, transfer or operate multiple Affiliate accounts without written approval.
- You are responsible for keeping account, contact and payout information accurate and current.
3. Qualified referrals and compensation
Affiliates earn $25.00 USD per qualified referral. A referral is "qualified" only when the referred person completes the qualifying enrollment action defined by the program, the referral is properly attributed to your Affiliate ID, the referral passes fraud and duplication screening, and any applicable review period has elapsed without cancellation, chargeback or reversal.
Referral compensation is never tied to the medication selected, prescriptions issued, treatment chosen, provider decisions, amount spent by a referred person, or any clinical outcome. Compensation is paid only on a qualified referral event as defined by the applicable program terms.
- Attribution is determined solely by Connect Med's server-side tracking records.
- Self-referrals, referrals of household members created to obtain compensation, incentivized sign-ups and automated or bot-generated traffic are not qualified referrals.
- Commissions are subject to hold, reversal or forfeiture in cases of fraud, policy violation, refund, cancellation or duplicate accounts.
4. Payouts
Commissions accrue in your partner dashboard and become payable once a referral is qualified and approved. Payouts are governed by the Payout & Tax Terms, including the accumulated balance threshold, payout schedule, holds and tax documentation requirements.
5. Affiliate obligations
- Promote only with approved messaging and comply with the Marketing Compliance Policy.
- Disclose your material connection to Connect Med in accordance with the FTC Disclosure Policy.
- Never provide medical advice, diagnose, recommend treatment or represent yourself as a Connect Med healthcare provider or employee.
- Never collect, request, store or transmit protected health information (PHI) from referred individuals.
- Comply with all applicable federal, state and local laws, including healthcare marketing, anti-kickback, fee-splitting, patient-brokering, telemarketing, anti-spam and advertising laws.
6. Prohibited conduct
- Paid search bidding on Connect Med brand terms, or domains, handles and profiles that imply you are Connect Med.
- Unsolicited bulk email, SMS or messaging, spam, cookie stuffing, malware, iframe injection or misleading redirects.
- Earnings guarantees, medical claims, cure or outcome claims, or fabricated testimonials.
- Coupon, cashback or incentive offers that are not authorized in writing by Connect Med.
7. Independent contractor status
You participate as an independent contractor. This Agreement does not create an employment, partnership, joint venture, agency or franchise relationship. See the Independent Contractor Notice.
8. Intellectual property
Connect Med grants you a limited, revocable, non-exclusive, non-transferable license to use Connect Med names, logos and approved marketing assets solely to promote the program during your participation. All goodwill inures to Connect Med. The license terminates automatically when your participation ends.
9. Term, suspension and termination
Either party may terminate at any time with written notice, including through the partner dashboard. Connect Med may suspend an account, place a payout hold or terminate immediately for suspected fraud, compliance risk or violation of this Agreement or any referenced policy. Upon termination you must stop using Connect Med marks and referral links. Qualified, approved and unreversed commissions earned before termination remain payable unless the termination results from fraud or a material compliance violation.
10. Disclaimers, liability and indemnity
The program is provided "as is" without warranties of any kind. Connect Med is not liable for indirect, incidental, special, consequential or punitive damages, and total liability under this Agreement will not exceed the commissions paid to you in the twelve months preceding the claim. You will indemnify and hold Connect Med harmless from claims arising out of your promotional activity, your statements, or your breach of this Agreement or applicable law.
11. General
This Agreement, together with the referenced policies, is the entire agreement between the parties. If any provision is unenforceable, the remainder stays in effect. Connect Med may assign this Agreement; you may not without written consent. Notices may be delivered electronically as described in the Electronic Communications Consent.
