- No misrepresentation of your relationship with Connect Med.

This site describes business partnership programs. It does not provide medical advice, diagnosis or treatment, and nothing on it creates a provider-patient relationship. Clinical eligibility and treatment decisions are made only by licensed providers.

Changes, governing law and termination
Connect Med may modify these Terms and will post the updated effective date. We may suspend or terminate access at any time. These Terms are governed by the laws of the State of Arizona, without regard to conflict-of-laws rules, and the parties consent to venue in Maricopa County, Arizona.
