Privacy Policy

How data is collected and used on the partner platform. Partners never receive PHI.

Scope

This Privacy Policy describes how Connect Med collects and uses information through the Connect Med business partnership website and partner dashboard. It does not cover the clinical services delivered by licensed providers, which are governed by separate patient-facing privacy notices and, where applicable, HIPAA.

Information we collect

  • Account information: name, email, phone, mailing address and account credentials.
  • Program information: application answers, business name, state, program tier, agreements accepted and acceptance timestamps.
  • Referral and attribution data: referral code used, landing page, source, medium, campaign and a non-identifying visitor hash.
  • Commission and payout data: qualified referral records, balances, payout status and tax form status.
  • Technical data: IP address, device and browser information, and log data used for security and fraud prevention.

Partners do not receive PHI

Partner dashboards show referral counts, statuses and compensation. They never expose patient identity, intake answers, clinical records, medications, diagnoses or any other protected health information.

How we use information

  • To operate enrollment, attribution, commission calculation and payouts.
  • To verify identity, prevent fraud and enforce program policies.
  • To provide support and send service and compliance communications.
  • To meet tax, accounting, legal and regulatory obligations.

Sharing

We share information with service providers that host the platform, process payments and provide analytics and communications, each bound by confidentiality obligations. We may disclose information when required by law, to protect rights and safety, or in connection with a corporate transaction. We do not sell partner personal information.

Cookies and attribution

Referral links set attribution records server-side and may store a cookie or similar identifier so a referral can be credited to the correct partner. You can control cookies through your browser, but disabling them may prevent referral attribution and dashboard functionality.

Retention and security

We retain account, commission and tax records for as long as needed to operate the program and to meet legal retention requirements. We use administrative, technical and physical safeguards, including access controls and row-level database security, to protect partner data. No system is completely secure.

Your choices and rights

You may access and update your account information in the partner dashboard, request a copy or deletion of your personal information, or opt out of non-essential email. Deletion requests are subject to records we must retain for tax, fraud-prevention and legal purposes. Residents of certain states may have additional rights; contact support@myconnectmed.com to exercise them.

Children

The partner platform is not directed to individuals under 18, and we do not knowingly collect their information.

Questions about this document, program eligibility or compliance obligations may be directed to Connect Med partner support at support@myconnectmed.com. Connect Med compliance administrators are the final authority on program eligibility, payout holds, commission reversals and program exclusions.
